Cascade AIDS Project is excited to announce that we are hiring skilled and team oriented CareLink Navigator. The CareLink Navigator works with staff and management of Cascade AIDS Project (CAP) to provide time limited, high quality, coordinated, strengths-based social services consistent with the agency’s mission to people living with HIV/AIDS (PLWHA) who are newly diagnosed and need additional support to engage in HIV related care, have not been in medical care for at least six months, and/or are releasing from incarceration and need assistance connecting to medical care, and other related support services. The CareLink Navigator assists participants with navigating the healthcare, mental healthcare, and substance use treatment systems as well as provides support in the form of referrals to housing and shelter resources as this is a critical component to their stability and medical engagement.

This is a 0.8 FTE role working at our Davis St. location in NW Portland. The schedule for this role is Hybrid at least 3 days in office; Mon-Thur 9:00am-5:00pm or Mon-Thur 9:00am-4:00pm and Fri 9:00am-1:00p.

Starting pay is $36,647.56 a year. The CareLink Navigator’s Pay increases with tenure and the top of the pay scale is $42,914.56. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • Bachelor’s Degree in human/social services field (social work, public or community health, psychology) or related field or equivalent experience.
  • A minimum of one-year experience providing case management services to people who have experienced houselessness, mental health and/or substance use needs.
  • Demonstrated computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet.
  • Excellent written and oral communication skills
  • Experience working with ethnic, racial, economic, and sexually diverse populations and a strong understanding of issues relevant to people living with HIV, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities that we serve.

What You’ll Do

  • Assessing participant needs
  • Providing information and referrals.
  • Developing individual goal plans and assisting participants in carrying out the activities to achieve their goals.
  • Completing forms and entering data into agency databases in a timely and accurate manner.
  • Working collaboratively with housing case managers, peer support specialist(s), and community partners.
  • Participating in additional department and agency activities and events, and other duties as assigned.
