General Function: 

Under the supervision of the Program Manager for Early Ready, Set, Go!, the Family Support Coordinator will provide child development services, social services, health services and parent enrollment services to families including home visits, advocacy and crisis intervention. The Family Support Coordinator will plan and implement a safe and developmentally appropriate experience for infants, toddlers and preschool age children, and home visits appropriate to the needs of each family in accordance with program guidelines.

Essential Duties and Responsibilities:

  • Provide child development, parent education and family support through home visits to families of infants, toddlers and preschool aged children
  • Plans for and facilitates parent-child interaction groups and parent support groups
  • Provides developmentally appropriate curriculum, and a caring, nurturing and soothing environment for children
  • Develops in partnership with parents an individual development plan for each child
  • Develops a family partnership agreement with each family and supports families in achieving goals
  • Provides social services to families including advocacy, emergency assistance and referrals to the community.
  • Assists families in meeting their health needs including identifying and establishing a medical home and the provision of well child care
  • Develops and maintains contacts with other social service agencies, health agencies and mental health agencies and public schools within the community
  • Attend regularly occurring supervision meetings with Program Manager, Program Staff, and Agency 
  • Organize and maintain program/families’ files and documentation and ensuring these files remain up to date
  • Works closely with e’RSG Program Coordinator to apply children screenings (ASQ-3 and ASQ-SE), and other assessments as needed.
  • Supports and manages program evaluation surveys
  • Works closely with e’RSG Program Coordinator to maintain up-to-date data entry into the Efforts to Outcomes (ETO), and ServicePoint databases.

 

 

 

Qualifications


Knowledge, Skills and Abilities Required:

  • Training and a minimum of 2 years of experience in social services, parent education, and social service program development
  • Ability to read, write and speak fluently in Spanish and English is required
  • Ability to provide strength-based resource and referral services to families
  • Experience working in a public-school setting and/or with the early learning community
  • Ability to recognize and manage critical situations including following mandatory reporting regulations and responsibilities
  • Model expected behavior for children and adults guided by appropriate child development
  • Ability and commitment to continuous quality improvements and innovation
  • Ability to understand and execute Ready, Set, Go! program philosophy in day to day duties
  • Experience working in collaboration with a variety of agencies and programs
  • Ability to work independently and to collaborate with a team

Education and Experience Requirements:

  • Certification, or an Associate degree, or higher education degree in Early Childhood Education or minimum of 2 years of classroom experience in Early Childhood Education or related and equivalent experience
  • Fluency in English and Spanish (read, write and speak) is required. Fluency will be determined upon hire.

Other Requirements:

Personal transportation allowing for travel within the Portland Metropolitan area in a timely manner.  If driving a personal vehicle must maintain a valid driver's license for the state of residency, personal auto liability insurance coverage and a driving record permitting coverage under the agency auto liability policy.

Work Environment:

Due to COVID-19 issues, some work may be performed remotely and this position will also be working in classrooms with families and students as required.

Generally, work is performed in an office environment, at community events, and/or at other locations as deemed necessary.  May require occasional early morning, weekend or evening work.  Position requires the ability to sit for extended periods as well as stand occasionally.  Position requires use of computer, telephone, fax machine, copier and may require accessing locations by use of stairs.
