Cascade AIDS Project in SW WA is excited to announce that we are hiring skilled and team-oriented Medical Case Manager II. The individual works on Cascade AIDS Project’s SW Washington team to provide individuals living with HIV and those at high risk of HIV through the continuum of HIV-related and other relevant systems of care. This role focuses on helping clients to create and maintain engagement in medical care and medication adherence to strive to achieve viral load suppression. This requires maintaining an awareness of cultural challenges and developing effective relationships with a diverse client population and community partners throughout the service and surrounding areas
This is a Full-Time role working at our SW WA location in Vancouver WA. The schedule for this role is a Hybrid role, Monday – Friday 8:30am-5:00pm
Starting pay is $26.93 an hour which is equivalent to just over $56,000 a year. The Medical Case Manager II’s Pay increases with tenure and the top of the pay scale is $30.68. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.
Who You Are
- Bachelor’s Degree in the social services field (Social Work, Psychology, Public or Community Health) with two (2) years’ professional experience providing direct case management services OR minimum of three (3) years’ professional experience providing direct case management services
- Prior experience providing case management services with knowledge of care coordination, client advocacy, and community service navigation.
- Proficiency with tech and tools and software, including Microsoft Office (Word, Excel, PowerPoint, Outlook) and willingness to learn new software
- Excellent written and oral communication skills
- Experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction
- Strong understanding of issues relevant to gay/bi/trans men, people living with HIV and AIDS, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery
What You’ll Do
- Provide comprehensive strength-based social services to PLWHA
- Guide and support a caseload of up to 85 individuals facing low and medium barriers to care with a focus on medical engagement and medication adherence
- Work in collaboration with medical providers, or other providers involved in client care, to provide outreach and coordinated comprehensive care to low and medium PLWHA with a focus on medical engagement
- Complete comprehensive assessments, goal planning, linkage to services, and advocacy for and with clients
- Maintain a comprehensive understanding of the statewide standards for HIV Medical Case Management and adhere to the requirements outlined in these standards.
- Makes appropriate referrals to mental health, substance abuse, HIV prevention, or other community partners as needed to support client self sufficiency
