Prism Health is excited to announce that we are hiring an experienced Outreach Worker/QMHA for Trans+ / Gender Diverse Services. The Outreach Worker/QMHA provides services to transgender identified individuals enrolled in Primary Care or Behavioral Health services who have been identified as having case management needs. The Outreach Worker/QMHA works to connect individuals to resources, promote greater independence, community integration, and recovery. This position will work collaboratively with the Peer Support Specialist to help resolve barriers and connect individuals with resources in the community.

The person in this position will be utilized between Prism Health and Providence Health & Services. This position includes travel throughout the service area for meetings and client home-visits. Must be available to work evenings and weekends for required agency events. This.

NOTE: This is a non-management, union-represented position. The compensation, benefits, and conditions of work for this position are collectively bargained.

COMPENSATION: $51,272 annually

Successful candidates will have:

  • Demonstrated written and verbal communication skills.
  • Demonstrated organizational and time management skills.
  • Ability to travel throughout the service area on a regular basis.
  • Demonstrated problem-solving skills and familiarity with de-escalation techniques.

MINIMUM QUALIFICATIONS:

  • QMHA credential through MHACBO
  • Bachelor’s degree in behavioral sciences field (social work, public or community health, psychology) or one year of relevant work, education, training, or experience.
  • Knowledgeable about the lived experience of experiencing mental health and/or substance use issues and willingness to mindfully share those experiences.
  • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance abuse.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities we serve.
  • Basic computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet.
  • Ability to work independently or within a team with accountability; exercise sound judgment and discretion, maintain healthy boundaries, respect, discretion, and professionalism always.
  • On occasion, be available to work evenings and weekends for required agency events.

PREFERRED QUALIFICATIONS:

  • CADC through MHACBO
  • Verbal and written fluency in English and Spanish.
  • Experience working with OCHIN Epic electronic health record (EHR).
  • Demonstrated commitment to advancing equity and inclusion in workplace or community settings.
