Location: Old Town Clinic (727 W. Burnside Street, Portland, 97209)

Schedule:

  • Daily: 7:30am - 5:00pm
  • Weekly: Monday-Tuesday, Thursday-Friday (Off-Day Wednesday; 4x9s)

MINIMUM QUALIFICATIONS (REQUIRED)

***To be considered for the Clinical Nurse (RN) position, candidates must ensure they meet the following minimum requirements***

    1. Required: Successful Completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course.
    2. Required: Current RN license in the State of Oregon.
    3. Required: 1 year experience in a clinic operation, preferably primary care, or equivalent.

SUMMARY: The Clinic Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self-care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information /education, referral and evaluation. The Clinic Nurse also supports his/her Primary Care Team during a period of rapid expansion and intake of new patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Client Care

  1. Supports Primary Care Team in rapid uptake of new patients by reviewing records of new patients, performing chart abstraction using standardized tools, doing initial triage and follow up with new patients who need more intensive care, as directed by their care plans
  2. Assesses clients needs, telephone response for urgent and non-urgent medical information.
  3. Provides instruction, training, and health information on self care and health maintenance as per the clients care plan.
  4. Responds to emergency events in the clinic.
  5. Administers medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, IV medication administration
  6. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

Care Management

  1. See patients who need nursing clinical interventions regularly as dictated by their care plans.
  2. Provide screening and assessment of on-going health issues (e.g. hypertension, congestive heart failure, asthma, COPD, wound management, and other chronic diseases.)
  3. Assure completion of referrals and coordinate care with clinical specialists.
  4. Recommends changes to the medications and medical care plan as needed, changes the nursing care plan as progress dictates.
  5. Determines frequency of visits based on clinical needs

Delegation

  1. Directs and assigns the care of individuals & groups of clients to members of the team based on the scope of practice and clinical skills needed by the clients.
  2. Assures client follow-up as needed for abnormal labs, specialty/referral reports, clinical treatment and prescriptions, etc.
  3. Teaches and observes team members’ specific clinical tasks of client care.
  4. Provides expertise and oversight for staff to which clinical care duties are assigned.
  5. Checks for competencies of team members as needed.

SKILLS AND ABILITIES:

  1. Working knowledge of principles, practices and procedures of clinic patient care
  2. Ability to anticipate patient and provider needs
  3. Working knowledge in how to access, interpret and apply computerized client data information
  4. Working knowledge of programs and services available to clients
  5. Ability to effectively screen and gather required information in a professional, culturally competent and customer service oriented manner
  6. Ability to communicate clearly and concisely, both orally and in writing
  7. Ability to establish and maintain cooperative working relationships with those contacted in the course of work
  8. Ability to be flexible and manage multiple priorities
  9. Ability to maintain composure and work objectively with clients who may be hostile or verbally abusive
