LifeWorks NW is a private, non-profit organization providing prevention, mental health, substance use treatment, and related social services to youths, adults, and older adults in over fifteen locations throughout Multnomah, Washington, and Clackamas counties. The Mental Health Therapist assesses and diagnoses children and adolescents ages 3 to 18 experiencing mental health issues and behavioral problems in school based programs as well as the Virginia Garcia medical clinics. They provide appropriate treatment and/or consultation with families, care providers, and community partners about treatment options.
As a behavioral health organization, LifeWorks NW employees are required to have had their COVID 19 vaccination (or have an approved medical/religious exemption) per State of Oregon rules. Proof of COVID-19 vaccination is required upon hire.

Pay/Benefits: $58,689 to $62,849 annual salary, plus full comprehensive benefits. $1,000 sign-on bonus.
QMHPs at this LifeWorks NW site qualify for the Oregon Health Care Provider Incentive Loan Repayment Program and/or the Oregon Behavioral Health Loan Repayment Program.
Essential Responsibilities: (Must be able to be performed with or without reasonable accommodation.)
- Meets performance standards for productivity, documentation (including progress notes, assessments, treatment plans and authorization requests) proficiency in clinical model and evidence based practices, quality of care (including engagement and retention), and teamwork as defined in the Performance Review.
- Continually increases level of cultural sensitivity, awareness and competency. Provides appropriate resources and support linkage and evaluation/treatment to clients.
- Performs client evaluation including; initial intake interviews, formulating diagnoses, and identifying appropriate treatment services. Notifies client and significant other(s) of diagnoses and initial recommendations. Completes a written narrative of the evaluation. Obtains necessary releases within specified timeline standards.
- Coordinates with Primary Care Physicians doing some work with the behavioral health model focusing on disease management.
- Collects supplementary information needed to assist client, such as medical records, or school reports. Analyzes the data such to identify symptoms and to diagnose the nature of clients' problems.
- Obtains and studies medical, psychological, social, and family histories by interviewing clients individually, in families, or in groups. Assesses their situations, capabilities, and problems to determine what services are required to meet their needs.
- Consults with parents, teachers, and other school personnel to determine causes of problems such as truancy and misbehavior in order to implement solutions.
- Develops therapeutic treatment plans based on clients' interests, abilities, strengths and needs. Treatment plans will be based upon clinical experience and knowledge, and will have measurable goals and objectives based on the diagnosis and will specify type, frequency, intensity, and duration of therapy. Treatment plans will be client driven and follow-ups will be completed on time, assessing the quantity and quality of services provided.
- Conducts individual, family and group therapy sessions based on current evidenced based practices e.g. collaborative problem solving, cognitive behavioral interventions, and solution focused therapy.
- Responsible for case management. Coordinates/collaborates with physicians, staff, families, caregivers, etc.
- Maintain awareness of community resources and provides referral services to clients and their families.
- Evaluates the effectiveness of counseling or treatment and the accuracy and completeness of diagnoses. Modifies plans and diagnoses as necessary. Uses program outcome measures as assigned.
- Maintains chart notes, obtains pertinent records from other treatment, and tracks client's involvement, follow-through, and progress in treatment activities. Documents and maintains client records to ensure compliance with agency and contract guidelines and confidentiality. Completes documentation in electronic health record system.
- Identifies cases involving domestic abuse or other family problems affecting students' development and determines whether clients should be counseled or referred to other specialists. Addresses legal issues, such as child abuse and discipline and assists with hearings.
- Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations.
- Provides training and consultation to school staff and community partners regarding issues of mental health, addiction and behavior management and prevention.
- Participates in operations meetings to help address and maintain sustainability of School Based Health Center model. Attends informational and clinical meetings which may include all-agency meetings, supervision meetings, team meetings, consultation meetings with psychiatrist, intra-agency trainings, and completes online trainings as assigned.
- Serve as back-up for other staff when needed.
- Check e-mail/voice mail daily/throughout the day. Responds to contacts within 48 hours for routine matter when working (urgent matters may require a more immediate response)
- Travels between sites or within the community
- Completes critical incident reports when indicated within 24 hours of a critical incident.
Providing Direct Client Care - Individual family and group therapy; case management; consultation; assessment; skills training; crisis intervention.
Assisting Clients - Responsible for rendering assistance in clean up duties involving the bodily fluids spill kit when providing services at LifeWorks NW sites.

- This position requires a QMHP credential either through the Mental Health & Addiction Certification Board of Oregon (MHACBO) or by obtaining and maintaining active internship or licensed with an eligible state licensing board. All prospective candidates must either have a valid MHACBO registration number or licensing intern number.
- Master's Degree in Psychology, Social Work or Mental Health Counseling or related mental health field (QMHP) may be required.
- Oregon license (i.e Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, Licensed Psychologist) or registered intern in process for license strongly preferred.
- Current CADC or eligibility for CADC within two years of preferred.
- Demonstrated ability to conduct an assessment, including identifying precipitating events, gathering histories of mental and physical health, alcohol and other drug use, past mental health services and criminal justice contacts, assessing family, social and working relationships and conducting a mental status examination; complete a five-axis DSM diagnosis; write and supervise the implementation of an Individual Service and Support Plan; and provide individual, family and/or group therapy within the scope of their training.
- More than 2 years experience required in a broad range of clinical interventions and mental health assessments specific to children, adolescents and families.
- Fluency in Spanish required.
- Demonstrated understanding of cultural values and norms. Knowledge of acculturation and generational issues within the Latino community.
- Extensive knowledge and understanding of resources and appropriate support linkage available within the community. Sensitivity and understanding of specific barriers which may lead to lack of access and engagement.
- Effective inner-cultural communication skills in order to advocate for culturally diverse clients.
- Demonstrated understanding of behavioral health interventions in support of medical diagnoses. Understanding of the difference between behavioral health and mental health models and ability to determine most appropriate model to use.
- Demonstrated ability in providing case management services and compliance with confidentially in accordance with federal, state and/or funding source requirements.
- Knowledge of community resources and treatment agencies enabling appropriate referrals for specific client needs.
- Knowledge of medications commonly used by client population.
- Proficient in or have a functional knowledge of Microsoft Outlook and Word. Must be able to master proprietary software including electronic health record system.
- May require access to a car and valid driver's license.
