Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty, and addictions. This position is a critical member of a multi-disciplinary team that provides comprehensive, intensive residential treatment for those who struggle with severe substance use disorder, The team works to address all treatment needs, including crisis management, substance use treatment, medication management, physical health treatment, mental health treatment, and accessing appropriate and affordable housing. This is a direct service position providing person-centered, trauma-informed care to clients experiencing mental health and substance use symptoms. This position has access to mental health billing and service records.

Salary pay starts at $65,000 annually and increases with professional experience and licensure/certifications!

Schedule: Monday-Friday 9:00a-5:00p

MINIMUM QUALIFICATIONS

  • Must meet qualifications for:
    • QMHP
    • CSWA
    • LPC-A or LPC
    • LMFT-A or LMFT
    • LCSW
  • CADC-R, at minimum, certification preferred, or the ability to obtain one within 1 year.
  • Experience in mental health and substance use treatment preferred.
  • Has the competencies necessary to:
    • Identify precipitating events.
    • Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services, and criminal justice contacts.
    • Assess family, social and work relationships.
    • Conduct a mental status assessment.
    • Write and implement or supervise implementation of a treatment plan.
    • Conduct and document a mental health assessment.
    • Provide mental health treatment and rehabilitative services within one's scope of practice.
  • Must meet CCC privileging requirements as required by FTCA
  • Must become a certified Director’s Custody designee in Multnomah County within the first 6 months of hire.
  • Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit. Must be approved by the Childcare Background Check Unit.
  • Have or able to obtain Adult and Child/Infant CPR Certification prior to start date.
  • The OAR prohibits substance use treatment program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Prior to employment, must be approved by the State of Oregon Employment Department Child Care Division. Must complete consent for criminal records identification check pursuant to ORS 181.537. This program receives funds from the Federal Child Care Development Fund block grant via a contract with the Child Care Division for Alcohol & Drug Abuse Programs.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. Must be able to climb stairs several times a day.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • If using a vehicle for agency use, must possess a current driver’s license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
  • Must adhere to agency’s non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Collect information regarding clients’ history through a combination of interviewing, records review, and consultation; after obtaining written consent.
  • Conduct and document substance use disorder & mental health assessments, including mental status exam, ASAM level of care, DSM diagnosis, and clinical formulation.
  • Understanding of how medication supports can be useful in the recovery process.
  • Develop strengths-based treatment plan and collaborative problem solving with clients.
  • Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual service plans for clients.
  • Share substance use and mental health expertise by working collaboratively with the multidisciplinary treatment team, modeling integrated treatment skills, and evidence-based principles and practice.
  • Support clients in accessing off-site appointments and collaboration meetings as necessary, to include court appearances.
  • Participate in direct treatment of clients by:
    • Attending clinical staff meetings and case conferences.
    • Providing substance use and mental health treatment groups for clients, including addiction as a disease process, recovery, Trauma Informed Care components, and life skills.
    • Providing one-on-one evidence-based practice counseling sessions
  • Facilitate mental health and substance use treatment groups for clients, including addiction as a disease process, recovery, Trauma Informed Care components, and life skills
  • Assess, triage and respond to mental health and substance abuse crisis in the most effective, efficient, and safe manner possible.
  • Participate in evaluating clients for risk of harm and initiate Directors Custody transport holds when appropriate.
  • Monitor and ensure the health and safety needs of clients are met.
  • Quickly and accurately assess the needs of individual clients and potential barriers to service.
  • Utilize Motivational Interviewing and trauma-informed treatment modalities to identify stages of change and develop corresponding stage wise treatment strategies.
  • Maintain client records as required by State, Federal, local and program standards and funding requirements.
  • Works within scope of training and experience and seeks consultation when appropriate.
  • Participate in regular supervision from the Clinical Supervisor, seeking additional supervision whenever necessary. Minimum requirement two hours of documented supervision per month.
  • Attend required meetings and trainings.
  • Attend all mandatory CCC training in a timely manner.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

DESCRIPTION OF OTHER DUTIES

  • Collaborate with partnering agencies, including DHS Child Welfare and the Department of Justice.
  • Provide testimony and advocacy with courts and other agencies.
  • Coordinate services with all members of client’s support network.
  • Participate in training opportunities to advance skills and knowledge in substance use disorders and mental health.
  • Participate in quality assurance and quality improvement activities as directed.
  • Assist with the development of an integrated treatment program.
  • Perform other duties as assigned.
