The Our House Residential Care Facility (RCF) is a 14-room facility with a team of expert clinicians who work with people living with HIV who are having difficulty managing independent living. By tailoring nursing, social services, and therapeutic everyday activities our clients and residents are able to define what ‘living well’ means to them. Our House a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented NHCP Outreach Case Worker. The Outreach Case Worker will be instrumental in the recruitment of new clients for NHCP. This position is split between the needs of program outreach via developing community relationships and connecting with new clients, and case management via working collaboratively with team members (Peer Recovery Mentors, Case Workers, and Registered Nurse) to provide individuals who are living with HIV, Substance Use Disorders, and Mental Health conditions with wrap around supports so they can live as independently and successfully as possible in their community.

This is a Full-Time role working at Our House location in SE Portland.

Starting pay is $25.04 an hour. The NHCP Outreach Case Worker’s Pay increases with tenure and the top of the pay scale is $28.79. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • Bachelor’s degree in social services or a related field.
  • A valid driver’s license, acceptable driving record, and ability to be insured by Our House’s auto insurer.
  • Representative of the knowledge, skill, and/or ability required.
  • Prior experience working with people in the field who have substance use disorders and/or Mental Health conditions.
  • Experience working in outreach or recruitment setting and demonstrates skills in networking.
  • Ability and comfort level to work with the needs of those that are houseless.

What You’ll Do

  • Engagement with other community providers to develop and maintain relationships that foster ongoing referral sources.
  • Engagement and connection with new clients
  • Provides community-based case management and skills training services to our clients.
  • Develops comprehensive wellness plans that are driven by the overall needs and interests of the individual to include physical, emotional, mental, and spiritual needs.
  • Maintains charting on treatment goals and progress notes.
  • Efficient time management between outreach and case management responsibilities.
