CCC’s Old Town Recovery Center is seeking a Utilization Review and Management Coordinator!
Schedule: Monday – Friday, 8:00am – 5:00pm
The Utilization Review and Management Coordinator is a key member of the Mental Health Services department, responsible for providing administrative coordination of utilization review and management and care transition activities.
Responsibilities Include:
- Identify clients who are eligible for utilization review through the use of reports, registries, medical records, and other sources.
- Obtain, process, and document health information related to client utilization. This may include securing medical records when appropriate, and documenting items in the electronic health record or client registries.
- Analyze client utilization data and create summary reports to assist the Utilization Review and Management team.
- Support client engagement in the utilization management and care transition processes, including:
- Perform telephone outreach;
- Schedule clients for appointments or other services as appropriate; and
- Complete documentation in electronic health records and client registries.
- Provide utilization review and management and care transition-related administrative support, including data entry and other administrative tasks as assigned.
- Participate in training opportunities to advance skills and knowledge in mental health, substance use disorder, and physical health.
- Work within the scope of training and experience, and seek consultation when appropriate.
Requirements:
- Bachelor’s degree in community or public health, social work, or another relevant field of study preferred.
- Certification as a Qualified Mental Health Associate; Certification as a traditional health worker such as a Peer Support Specialist, Peer Wellness Specialist, or Personal Health Navigator strongly preferred.
- Must have at least 1 year (2 preferred) of experience in social services or safety net setting in a client-facing support role.
- 1-2 years of experience working in a mental health care setting in a client-facing support role such as case management preferred.
- Intermediate computer skills and high proficiency in Microsoft Office Suite required.
- Experience with electronic medical records, patient registries, or other clinical applications preferred.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
