Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Housing Navigator. The Housing Navigator works with the staff and management of Cascade AIDS Project (CAP) to provide high-quality, coordinated, strengths-based social services, consistent with the agency’s mission, to people living with HIV (PLWH). The Housing Navigator provides housing case management, advocacy on behalf of clients, goal planning, information and referral services, and assists clients in navigating various systems of care: medical, mental health, substance use, social services, etc. The individual will work across agencies and systems to ensure access to services through CAP and other social service providers.

This is a Full-Time role working at our Davis St. location in NW Portland. The schedule for this role is a Hybrid Monday-Friday 8:30am-5:00pm

Starting pay for this position is $47,370.59. The Housing Navigators pay increases with tenure and the top of the pay scale is $55,170. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

Who You Are

  • Bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field or two years’ experience relevant to the position
  • Prior experience providing case management (or similar) services
  • Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet
  • Excellent written and oral communication skills.
  • Successful experience working with ethnic, racial, economic and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction
  • Demonstrated ability to effectively collaborate with community stakeholders

What You’ll Do

  • Provide housing placement, Medical Motel placement, supportive case management, eviction prevention support and information and referral with and on behalf of clients who are homeless or at risk of becoming homeless using a supportive strengths-based model that promotes client self-determination and independence
  • Work in collaboration with medical case managers, medical providers, or other providers involved in client care, to provide coordinated comprehensive care to PLWHA with a focus on housing stability and engagement in HIV care.
  • Carry an active client panel of approximately 30 program eligible clients, assisting with eviction prevention, housing planning, advocacy, mediation, and information and referral.
  • Complete comprehensive housing assessments, goal planning, linkage to services, and advocacy for and with clients.
  • Assist clients with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocate for clients with prospective landlords
  • Accompany clients to appointments in the community to apply for housing and to reduce barriers to accessing housing
